Use this form to submit a complaint about a Wisconsin election official.  You may also use the attached PDF form at the bottom of this page if you wish to print it out, fill it in and mail it to the Government Accountability Board.  Please give us as much contact information as possible if you would like us to follow-up on your complaint.
