Getting on the Ballot

Candidates for local offices must file ballot access forms with their local clerk.  The forms are all available from the Government Accountability Board.  Click the heading below for a page with the appropriate documents. 

Partisan Ballot Access Documents

  • Sheriff
  • County Clerk
  • County Treasurer
  • Clerk of Circuit Court
  • Coroner
  • Register of Deeds

Non-Partisan Ballot Access Documents

  • County Executive
  • County Supervisor
  • Municipal Offices (including mayor, alderperson, trustee, supervisor, municipal clerk and treasurer, constable, municipal judge, assessor)
  • Multi-Jurisdictional Municipal Judge
